• Forms & Policies

    If you are a new client, please complete the Mindful Counseling Center Forms Package prior to your first counseling session (within at least 24 hours of your appointment is preferred).

    Fill out the forms package online by clicking below. Those coming in for an in-person appointment also have the option to fill out the forms in the office prior to their first session. Give yourself at least 15 minutes to complete the forms.

    Here is information about our practice to review:  To keep up to date on practice policies, periodically check to see if any of the below documents have been added or revised after the date you completed the intake forms packet. If so, it is recommended you review them.

    If you think it would be helpful for your therapist to reach out to one or more of your other health providers, in order to enhance your sessions, fill out the following authorization for release of information form.

    Also note that there is a binder in the Mindful Counseling Center’s office containing our policies and other practice information. If you are visiting the office and can’t find it, ask us for help.
